Inspector Notes

An unannounced, on-site renewal inspection was initiated on 05/29/2025 and completed on 05/30/2025, as a part of the licensure period. The on-site inspection began at 9:45am and ended at 10:50am. The inspector reviewed compliance in the areas listed above. There were four children present and one caregiver; the point total was three. The inspector reviewed five children?s records, one caregiver, and one household member's records on-site. Additional documentation was reviewed electronically on 05/30/2025. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening.

An updated TB screening for Caregiver #1 was not completed and was overdue by 29 days.
Plan of Correction: The updated TB will be obtained as soon as possible.
Standard 8VAC20-800-830-B
Shelter-in-place procedures shall be practiced a minimum of twice per year.

The shelter-in-place procedures were only practiced once in 2024.
Plan of Correction: In the future, shelter-in-place procedures will be practiced at least twice per year.
