Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 05/28/2021 concluded on 06/03/2021. The in-charge person was contacted by telephone to initiate the inspection. There were 15 children present and 4 staff. The inspector emailed the in-charge person a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on a record review the center did not ensure each child had a physical examination under the direction of a physician.
Evidence:
Child #2, enrolled on 03/08/21, did not have documentation of having a physical examination.
Plan of Correction: The following corrective action was provided by the owner during the exit interview:
The owner stated she will contact the parent to obtain a copy of the physical.

Based on a record review the center did not have documentation of annual approval from the health department.
Evidence:
The last health inspection at the center's facility was dated 05/09/2019.
Plan of Correction: The following corrective action was provided by the owner during the exit interview:
The owner stated she has contacted the health department and an inspection is scheduled this week.
