Inspector Notes

An unannounced monitoringinspection was completed on 06/05/2025 as a part of the licensure period. The on-site inspection began at 1:00pm and ended at 2:00pm. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. There were 15 children present and 2 staff. The inspector reviewed 3 children?s records and 3 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.011-F
The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.

The finding of the last inspection were not posted on the licensed premises.
Plan of Correction: The last inspection will be posted in the front lobby until the current inspection is finalized.
Standard 8VAC20-780-350-M
One staff member shall not supervise more than one room or area during rest time.
Staff A was supervising 15 children spread out in 3 different classrooms.
Plan of Correction: During nap children will be spread out in rooms and the director will ensure staff supervise no more than 1 classroom.
Standard 8VAC20-780-560-F
When centers choose to provide meals or snacks, a menu listing foods to be served for meals and snacks during the current week be dated, posted in a conspicuous location and list substituted foods.

The center did not have a current menu posted. The menu was dated for the week of May 26th.
Plan of Correction: This was an oversight. Administration took the menu down to review and forgot to post it back up.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all required information.
The record for child #3 did not contain addresses for the 2 emergency contacts listed on the child's application.
Plan of Correction: Emergency contact addresses will be obtained form the child's parent and documented in the child's record.
