Inspector Notes

A complaint investigation was conducted with the local agency regarding forbidden action: physical punishment was initiated on September 28, 2021 and concluded on October 14, 2021. Remote staff interviews were conducted. An in-person interview was also conducted on October 12, 2021. Video recording and documentation was reviewed.
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

Based upon the information gathered during the investigation, the self-reported incident regarding physical punishment was determined to be valid.

Based on review of video recording and interviews conducted, a staff used the forbidden action of physical punishment of a child to correct the behavior of the child. Evidence: On September 27, 2021 staff A held her right foot over child 1's head and while the child lay on a cot. The staff used that same foot to roughly move the cot that the child lay on to another position.
