Inspector Notes
An unannounced monitoring inspection was initiated on 08/13/24 and completed on 08/19/24 in response to a self-report received by the licensing office on 07/26/24 relating to behavioral guidance and forbidden action. The inspector reviewed one child's record and one staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/07/24. A POC submitted after this date will not appear on the public website.
Violations
2On July 25, 2024, Staff #1 grabbed Child #1 (10-year-old) by the shoulders, threw them out of a chair and into a bookshelf after Child #1 was accused of pulling Staff #1?s hair.
Additional Correction - All staff members must follow the Zero Tolerance policy that COA developed to ensure each child's safety is always top priority. The director assigned a CCEI training to all teachers who work with school age children (GUI108-Practicing Positive Guidance with Preschool and School Age Children) and the training has been completed. Proof of the completed trainings are attached."
On July 25, 2024, Child #1 (10-year-old) was accused of pulling Staff #1?s hair. Staff #1 was observed by another staff member saying to Child #1 ?do not touch me ugly little girl,? grabbing the child by the shoulders, and throwing them out of a chair and into a bookshelf.
