Inspector Notes

An unannounced monitoring inspection was initiated on and completed on March 11, 2024 with a local agency in response to a self-report received by the licensing office on March 5, 2025 relating to supervision and forbidden actions. The inspector reviewed one child?s record, and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. When staff are supervising children, they shall always ensure their care, protection, and guidance. On 03/04/2025, according to staff, Child A (13 months old) was fussy, cried continuously, and refused to eat throughout the day. The Daily Sheet indicated that Child A did not eat breakfast at 9:37 am, nor lunch at 11:19am and 2:22pm. A mark was noticed on the child?s forehead by the classroom staff. The parent arrived for pickup at 2:47 pm and was informed about the child?s fussiness. The parent observed injuries on Child A while in the car seat. At 3:29pm, Child A was seen by a physician. The physician noted multiple injuries including scratches, abrasions, and bumps on the child?s chest, abdomen, scalp, neck, ear and forehead. Photographs documented the injuries. The parent stated that the center did not provide a phone call or incident report regarding the child?s behavior and injuries. After interviewing staff, reviewing documents, and making observations, the local department of social services determined the investigation was found for physical abuse.
