During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced inspection was initiated on 12/04/25 and completed on 01/15/26, in response to a complaint received by the licensing office on 11/14/25 relating to allegations of staffing and supervision. The inspector reviewed four staff records on-site on 12/09/25. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
