The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.

Staff #1 (employed over 3 years) and currently working in the center completed the course 3 years late.
Plan of Correction: Staff member has completed the orientation course.
Standard 8VAC20-780-240-I
The center shall keep documentation of orientation training for each staff member with the required information.

The record for Staff #1 (employed over 3 years) did not include documentation of the training topics, training delivery method, the individual conducting the training, and the date of training for orientation.
Plan of Correction: Staff member #1 received updated training on school policies & protocols as required by BeanTree Learning.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On January 23, 2026, Staff #1 (employed over 3 years) did not ensure Child #1?s care and protection when they served Child #1 (23 months old) a known and documented allergen, requiring them to seek outside medical treatment.
Plan of Correction: Staff member was placed on 90-day probation. Reviewed allergy and food protocols with staff member.

Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications.

Staff #1, who did not meet lead teacher qualifications and was alone with a group of children in Classroom #4 for one week while Staff #8 was absent.
Plan of Correction: Staff member #1 will be getting additional training to meet lead teacher qualifications.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and include the required information.

On January 23, 2026, Child #1 (23 months old) ingested an allergen and had to seek outside medical attention. The center did not create a written record of the incident until January 29, 2026.
Plan of Correction: Director wrote an official incident report the day we returned from two inclement weather closure days.
Standard 8VAC20-780-550-Q
A parent shall be notified immediately of any confirmed or suspected allergic reaction and the ingestion of or contact with any food in the written care plan even if a reaction did not occur.

On January 23, 2026, Child #1 (23 months old), was served a cupcake containing a documented allergen. The parents were not notified and the child had a reaction 3 hours later and had to seek outside medical treatment.
Plan of Correction: The child did not have the reaction until they left school and we did not know they had the cupcake.
Standard 8VAC20-780-560-P
A child with a diagnosed food allergy shall not be served any food identified in the written care plan.

Staff #1 (employed over 3 years) served Child #1 (23 months old), with a diagnosed food allergy, a cupcake containing a food identified in their written care plan.
Plan of Correction: Staff member was talked to about food protocols. Also discussed checking ingredients and calling a director for prior approval.
