Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 04/12/2021 and concluded on 04/19/2021. The provider was contacted by email to initiate the inspection. There were 6 children present and 2 caregivers. The required documentation was viewed during the remote inspection.. The Inspector reviewed 3 children?s records submitted by the provider to ensure documentation was complete. Caregiver training was reviewed.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard 22VAC40-111-100-A
Based upon review of three children's records and provider interview, the provider has not ensured that documentation of a physical examination by or under the direction of a physician prior to a child's attendance is obtained prior to the child/s attendance or within 30 days after the fist day of attendance.
Evidence:
1. The record provided for child 3 indicated an attendance start date of 2/29/2021. Documentation of a physical examination was not in the child's file.
2. The provider verified that she did not have documentation of the child's physical examination.
Plan of Correction: The provider responded with the following:
The parent will be notified today that a copy of the child's physical examination must be submitted. Child 3 is scheduled for the one year doctor's examination so documentation will be obtained.
