Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety (SHSI) inspection was initiated and concluded on 12/02/2021. There were 4 children present with 5 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-665-500-F
Based on documentation review, the center did not maintain a current written list of all children's allergies, sensitivities, and dietary restrictions.
Evidence:
1. The center had a list of children's allergies, sensitivities and dietary restriction, but it did not list Child #1's diagnosed food allergy.
Plan of Correction: Once parent provides center with doctors documentation will add child's information to the food allergy list.
Standard 22VAC40-665-520-B-11
Based on record review, the center did not have a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1. Child #1 (start date 11/30/2021) had a diagnosed food allergy and the center did not have a written allergy care plan on file during the inspection.
Plan of Correction: Once parent provides center with doctors documentation will be able to have the allergy care plan.

Based on record review, two of three children's records did not have the address for each custodial parent's place of employment.
Evidence:
1. Child #1 and Child #2 did not have the address for each custodial parent's place of employment.
Plan of Correction: Will have parent put unable to disclose in the employment section.
