Inspector Notes

A monitoring inspection was conducted on 02/22/2024 between the hours of approximately 11:00 am and 12:00 pm. There were 6 children present, ranging in ages from 6 months to 3 years, with 2 caregivers supervising. Children were observed while playing with toys and eating lunch.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. There were no medications on site today. A total of 6 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 22.1-289.011-F
Based on review of required posted documents, the Provider did not ensure that all required documents related to the terms of the license were posted on the premises of the program.
Evidence:
1) On the date of inspection, the findings of the most recent inspection of the facility (07/2023) were not posted.
Plan of Correction: I already posted.
Standard 8VAC20-800-100-A
Based on record review, the Provider did not ensure that each child have a physical examination by or under the direction of a physician within 30 days after the first day of attendance.
Evidence:
1) The record for Child #2 (start date 06/21/2023) did not contain the results of a physical examination.
Plan of Correction: I asked the parent to provide ma as soon as possible the physical.
Standard 8VAC20-800-50-A
Based on documentation review, the family day home (fdh) did not keep an accurate written record of children in attendance each day.
Evidence:
1) At the time of entrance, 6 children were in care in the fdh. No attendance record was available for review to document the children that were in care.
Plan of Correction: I fixed already.
Standard 8VAC20-800-780-B
Based on record review, the provider did not review with the parent, annually, the emergency contact information to ensure the information is correct and obtain the parent's signed acknowledgment of the review.
Evidence:
1) The most recent reviews of the emergency information in the records for Child #4, Child #5 and Child #6 were dated 01/13/2023.
Plan of Correction: I asked the parents to update the emergency contact of Child #4, #5 and #6.
