Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A code compliance inspection was initiated on May 5, 2021 and concluded on May 6, 2021. The staff person in charge was contacted by telephone to initiate the inspection. There were eight children present and one staff. The inspector emailed the staff person in charge a list of items required to complete the inspection. The Inspector reviewed two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the facility.

Based on record review, in one of two staff records reviewed, the center did not a copy of the results of a criminal history record information check and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:

1. Staff #2 indicated on the sworn statement as to living in two different states outside of the state of Virginia within the last five years.

2. The staff person in charge confirmed that in the record for Staff #2, there were no results of an out of state criminal record clearance from one state and no results of an out of state child abuse and neglect registry or equivalent registry from the second state.
Plan of Correction: We are in the process of getting the signatures from Staff #2 to send out the paperwork for NC and CA.
