Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview and a virtual tour of the program.

A (monitoring/renewal/complaint) inspection was conducted on 3/31/2022. The provider was contacted by telephone and a virtual inspection was conducted. There were 7 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 children's records and 2 household records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-140-B-3
Based upon review of caregiver records, the provider has not ensured that each caregiver has current certification in cardiopulmonary resuscitation (CPR).
Evidence:
The CPR certification obtained by caregivers 1 and 2 through EMS Safety was an online course that did not include an in-person demonstration of skills.

In-person demonstration/certification of FA/CPR skills will be obtained.

Based upon review of caregiver records, the provider has not ensured that each caregiver has current certification in first aid.
Evidence:
The first aid certification obtained by caregivers 1 and 2 through EMS Safety was an online course that did not include an in-person demonstration of skills.

Based upon review of household records, the provider has not ensured that tuberculosis screenings for household members are obtained at least every two years.
Evidence;
The most recent tuberculosis screening provided for household member 1 is dated 1/3/2020.

Household member 1 will obtain a tuberculosis screening.

Based upon review of children's records, the provider has not ensured that each record contains the parent's signed acknowledgement of the receipt of whether or not there is liability insurance.
Evidence:
The parent of child 1 did not sign the acknowledgement form as to whether or not the provider has liability insurance.
