Evidence:
1.The records reviewed for child 1, 2, and 3 did not include the policy numbers of their medical insurance coverage.
2.The provider confirmed that the three children?s records had no medical policy numbers included in the space provided on the enrollment form.

Evidence:
There were two bottle fed infants in care. Staff 2 was feeding an infant a bottle during the inspection. The bottle was not labeled with the child?s full name and the date.
