Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol including telephone contacts, documents review, interviews and remote inspection.

A monitoring inspection was conducted on February 25, 2022 and concluded on February 28, 2022. The provider was contacted by telephone and a remote inspection was conducted. There were four children present, age ten months through four years of age, for a total of ten points, with one staff supervising. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, special care and emergencies, preventing the spread of disease and nutrition. Two children's records, one staff record and one household member record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on review of household member records, the provider failed to obtain updated central registry background checks every five years as required. Evidence: Household member #1's most recent Central Registry background check was dated 6/22/2016.
Plan of Correction: An updated background check will be done as soon as possible.
Standard 8VAC20-800-180-A
Based on review of documentation and interview with the provider, the provider did not obtain documentation of an updated Tuberculosis (TB) screening/test every two years as required. Evidence: The most recent TB test in the file was dated 6/3/2019.
