A focused non mandated monitoring inspection was conducted on 8/23/2023 regarding a self reported incidence involving sight and sound supervision. The center was presented with a violation notice along with this inspection summary. The ongoing compliance with the standards is the responsibility of the licensee,

Standard 8VAC20-780-340-F
Based on the interviews with staff and parents the center failed to keep a child, age 2, in sight and sound supervision.
Evidence: On Friday 8/18/23 at 15:15 a child in the two year old room was left alone in the room for about 2 minutes. Staff 1, counted the children and left the room to go to the large playground at the other end of the center. All 9 children in care were accounted for. according to the child supervision record. When she reached the door to the large playground she did a name to face check and realized one child had stayed behind. She turned around and watched the director and the father enter the classroom. The child was found in the classroom and it appeared that he had gone to the bathroom. The child was not in distress when he was found by the parent and director.
Plan of Correction: We will make sure that children are in sight and sound supervision through the following steps: We will always keep all children together and account for them using our CSR. Any time a group leaves a classroom the teacher or supervising staff member will complete a transition prior to leaving the location and once they have made it to their next destination. This will prevent children from being left by themselves in the future. The classroom teachers will be responsible for implementing the preventative measures with a retraining of all staff by 8/24. The Management team will ensure that teachers are doing this properly by completing at least 2 manager checks throughout the day which will be documented on the classroom CSR, as well as auditing CSRs daily to check for accuracy. By 8/25 the management team will observe one transition per classroom to provide any guidance needed, moving forward the management team will observe one transition in each classroom per week.

Based on the review of the child supervision records and interview with staff, the center failed to have a ratio of 1:8 in the 2 year old room on 8/18/2023.
Evidence: The center had 9, 2 year old children in a grouping with 1 staff from 14:45- 15:15 on 8/18/2023.
Plan of Correction: Teachers have been told that it is their responsibility to reach out for help when they are out of ratio. The management team will support and maintain required ratios through management checks of the CSRs.
