Inspector Notes

An unannounced renewal inspection was conducted today from 11:40am - 2:10pm. 84 children were in attendance with 16 staff directly supervising in 9 groupings. Children were observed eating lunch, engaging in circle time, dancing, playing and napping. Lunch was baked beans, coleslaw, fruit and corn muffin. Lots of teacher and child interaction was observed. 6 children's records and 5 staff records were reviewed during todays inspection. A violation notice was given to the center along with this inspection summary. Based on record review, the center failed to get TB results for 1 staff
Evidence: One staff, staff #1,out of five staff records reviewed was missing documentation of a negative test or screening for tuberculosis, staff #1 submitted a chest x-ray, the chest x-ray report mentioned GERD and chest pains but not TB results. .
Plan of Correction: We are sending the staff to get TB done. We thought what they gave us was ok.

Based on observation, equiptment of the center, inside, was not maintained in a clean, safe and operable condition.
Evidence: A toilet lid was missing from the toilet in the 2 year old classroom.
Plan of Correction: We have contacted maintenance to get it fixed. We will cover the top of the toilet until it is fixed.

Based on observation, hazardous substances such as cleaning materials were not kept in a locked place.
Evidence:In the two year old classroom, two upper cabinets containing cleaning sprays had locking mechanisms that were not engaged.
Plan of Correction: Fixed during inspection.
