During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 06/09/2025 and completed on 06/14/2025. The on-site inspection began at 9:20 AM and ended at 10:40 AM. The inspector reviewed compliance in the areas listed above. There were 50 children present and 4 staff. The inspector reviewed 5 children?s records, 5 staff on-site. This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/20/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

Staff #1 updated screening was due five months ago. The updated screenings have not yet been completed.
Plan of Correction: Staff # 1 has an appointment for TB test today.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

There was a medal plate on the edge of the door that had sharp edges.
Plan of Correction: The metal plate has removed from the door.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

A container of sanitizing Clorox wipes with warning labels, ?Keep out of reach of children,? were unlocked at the front desk where children enter the building and accessible to the children.
Plan of Correction: Staff has been made aware about the cleaning materials and was corrected.
Standard 8VAC20-780-60-A-8
A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child #2, who has been enrolled for six weeks, has a diagnosed food allergy and did not have a written care plan in their file.
Plan of Correction: Child # 2 has an Emergency Plan now.
