During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site renewal inspection was initiated on 09/17/2025 and completed on 09/17/2025. The inspection began at 10:15 am and ended at 11:20 am. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 caregivers; the point total was 10. The inspector reviewed 4 children?s records and 2 caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/01/2025. A POC submitted after this date will not appear on the public website.
Violations
6The most recent central registry check and criminal history check for household member #1 was overdue for over one year and nine months.
Household member #1 did not have a two year updated TB test or screening available.
Provider #1 did not have 16 hours of annual training hours.
The provider did not have a smoke detector hooked up in the basement level of the home.
Child #1 did not have the address and phone number listed for their emergency contacts.
Child #2 had an annual update that was overdue by five months.
Child #3 had an annual update that was overdue by two months.
Child #4 had an annual update that was overdue by one week.
