Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts and documents review.

A monitoring inspection was initiated on 9/17/2021 and concluded on 9/17/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 12 children present, ranging in ages from 4 months to 3 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 3 household member records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-111-180-A
The provider did not obtain for each caregiver a current Report of Tuberculosis (TB) Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Evidence:
Caregiver #2's record contains documentation of a TB test/screening that expired on 8/9/2020.
Plan of Correction: Caregiver will update information.
