Inspector Notes

An unannounced renewal inspection was conducted on 9/12/2022 between 10:30am and 12:00pm. There were 12 children present with the Provider and 2 Assistants for a total of 28 points within the supervision guidelines. The children were observed engaging in water play activities outside. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children, household members, and staff records, evacuation drills, and attendance were observed during this inspection. Thank you for your cooperation. Based on record review and interview, the provider did not obtain for each caregiver a current report of tuberculosis screening, every two years from the date of the first screening.

Evidence: Caregiver #3 tuberculosis screening expired on 8/24/2022.

Based on observation, not all small electrical appliances were unplugged while not in use.

Evidence: A toaster machine was plugged in and unattended in the kitchen area.
Plan of Correction: Unplugged whenever not in use.

Based on record review and interview, the provider did not review the emergency plan at least annually and update the plan as needed.

Evidence: The emergency plan was last reviewed on 6/20/2020.
