During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site code compliance inspection was initiated on 03/30/26 and completed on 03/31/26. The on-site inspection began at 9:40am and ended at 9:55am. The inspector reviewed compliance in the areas listed above. There was one child present with two staff. The
inspector reviewed four children?s records on-site. Two staff records were reviewed electronically on 03/31/26. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliances with applicable code sections, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 04/14/26. A POC submitted after this date will not appear on the public website.

Each person in a supervisory position should be certified by a practicing physician or physician assistant to be free from any disability which would prevent them from caring for children under their supervision.

The staff health report for Staff #2 was obtained five months late.
Plan of Correction: Per the Center: "Staff Health Report was completed in January. Future staff health report will be mandated before employment begins."

(Repeat Violation) The center must obtain a completed national criminal background check prior to the employee's first day of employment.

Staff #2, who has been employed for over seven months, did not have a completed national criminal background check. Staff #2 was working alone with children.
Plan of Correction: Per the Center: "Background check was submitted immediately after inspection. (Though it does not excuse, [management] known staff member well since birth).

The center must request a search of the central registry prior to the employee's first day of employment.

Staff #2, who has been employed for over seven months, did not have a central registry search requested. Staff #2 was working alone with children.
