Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/25/2026, as a part of the licensure period. The on-site inspection began at 11:05 am and ended at 11:45 am. The inspector reviewed compliance in the areas listed above. There were 8 children present and 2 caregivers. The inspector reviewed 3 children?s records and 2 caregiver/household member(s) records on-site.

This inspection included document review, tour of the program, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-90-C
The family day home shall obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements according to subsection B of this section: every 6 months.

Child A did not have documentation of 18 month immunizations that are over due by three months.
Plan of Correction: I will ask mom . She was sick last appointment.
