Inspector Notes

A monitoring inspection was conducted on April 20, 2023 from 1:00 pm to 3:15 pm. There were six children present, ranging from 11 months to four years of age (total of 16 points) with one staff supervising. The provider's record and a total of five children records were reviewed.

The inspector reviewed compliance in the areas of administration, care of children, physical plant, physical equipment and environment, staffing and supervision, programming, special care and emergencies and nutrition.

Medication is not administered. The provider home is equipped with toys and supplies and items were available to the children. The provider's interactions with the children were positive.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not
acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Based on observation, the provider failed to wash her hands with liquid soap and warm running water after diapering an infant.

Evidence:
During the inspection the provider was observed diapering an infant.

Based on observation the provider failed to ensure that children's hands are washed with liquid soap and warm running water after diapering.

Evidence:
During the inspection, the provider was observed diapering an 11 month old infant and immediately after diapering the infant, the provider placed the infant in a bouncy seat.

The infant's hands were not washed.

Based on observation, the diapering surface was not cleaned and sanitized after use.

Evidence: The provider was observed changing the diaper of one infant and did not clean and sanitize the diapering table.

The provider placed the blankets of 4 other children on the diapering table immediately after changing the infant.

The provider acknowledged during the inspection that the diapering observed was "unsanitary" and that she would wash the blankets on this date.
