Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on October 23, 2024. The on-site inspection began at 11:00am and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There were two children present and one caregiver; the point total was five points. The inspector reviewed five children?s records and one caregiver/household member(s) records on-site.

This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. The record for Child #5 did not contain documentation of a physical examination by or under the direction of a physician. Child #5 has been attending the family day home for six weeks.
Plan of Correction: The child just went to the doctor. I will ask the parent to a copy of the child's physical exam.
Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. The record for Provider #1 contained documentation of a TB screening obtained 25 months ago.
Plan of Correction: I will make an appointment to get an updated TB screening.
Standard 8VAC20-800-60-B
REPEAT VIOLATION

A child?s record shall contain documentation of the review of the child?s emergency contact information as required by 8VAC20-800-780 B. The records for Child #1, Child #2 and Child #4 documented the last review of the children?s emergency contact was over 17 months.
Plan of Correction: I will keep out the children's records and have the families review them today when they pick up.
