During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 04/23/2025. The on-site inspection began at 11:30am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were three children present and one caregiver; the point total was seven. The inspector reviewed five children?s records and one caregiver?s records on-site.

This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-60-B
Children's records are required to contain name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. Children?s records are also required to contain documentation of an annual review of the child's emergency contact information as required by 8VAC20-800-780 B. The records for Child #2 and Child #5 was missing the information for one of the two emergency contacts. Child #2 has been attending the family day home for more than three years. Child #5 has been attending the family day home for five months. The record for Child #1 was documented as being last reviewed over one year and seven months ago. The record for Child #2 was documented as being last reviewed over two years ago.
Plan of Correction: I will request the required information when the families pick up today.

Medication, except for those prescriptions designated otherwise by a written physician's order, including refrigerated medication and medications for caregivers and household members, shall be kept in a locked place using a safe locking method that prevents access by children. In the bathroom used by children, a prescription and over the counter medication were sitting on a shelf, out of reach of children.
