Inspector Notes

A monitoring inspection was conducted on 8/31/22 from 11:00am to 12:15pm. There were 10 children (36 points) present, with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. The children were observed playing with toys, washings hands before meals and eating lunch brought from home. Infant bottles, the first aid kit and attendance were reviewed. A total of 3 children's records, 3 caregiver's records and 2 household member's records were reviewed. Emergency drills were current.

Based on record review, the provider did not obtain a subsequent tuberculosis (TB) screening for each adult household member, every two years.
Evidence: Household Member # 1's most recent TB screening expired on 2/11/22.
Plan of Correction: The TB screening will be updated.
Standard 8VAC20-800-60-B
Based on records review, children's records did not have complete information.
Evidence: Child # 2's record was missing the authorization for emergency medical care.
Plan of Correction: The missing information will be obtained from the parents.
Standard 8VAC20-800-90-A
Based on records review the provider did not obtain documentation of child's immunization record prior to the first day of attendance.
Evidence: Child # 1's first day of attendance was 6/6/22. Child # 1's immunization record was not obtained to this date.
Plan of Correction: Immunization records will be obtained from the parents.
Standard 8VAC20-800-960-E
Based on observation, bottles were not labeled with children's full names and the date.
Evidence:
1. Two bottles for Child # 1's were not labeled with name and date.
2. One bottle for Child # 2's bottle was not dated.
Plan of Correction: Bottles will be checked daily to ensure the proper labeling.
