Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 07/29/2024 and completed on 07/29/2024, as a part of the licensure period. The on-site inspection began at 12:31pm and ended at 2:13pm. The inspector reviewed compliance in the areas listed above. There were 9 children present and 3 caregivers; the point total was 26. The inspector reviewed 6 children?s records, 3 caregiver records and 3 household member records on-site. This inspection included document review and a tour of the family day home.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/05/2024. A POC submitted after this date will not appear on the public website.
Violations
5Based on record review and interview, the provider did not obtain results of a fingerprint based criminal record check (CRC) for an employee prior to the first day of employment.
Evidence:
Caregiver #2 hired in February, 2024 but fingerprint criminal record check results were obtained on 02/28/2024.
Evidence:
Household Member #1's criminal background check record expired March 2023.
Based on record review and interview, provider did not complete a reference for a caregiver prior to employment.
Evidence:
Caregiver #2 hired in February, 2024 but one reference check was completed a day after hire date.
Evidence:
1. Child #2 is 3 years old and was observed using a crib for napping.
2. Provider stated the child was placed in the crib because he walks around during nap.
Evidence:
1. Child #1 has been enrolled since May, 2024 and was missing proof of identity.
2. Child #2 & Child #4's annual review of emergency contacts was last completed on 2/01/2023.
3. Child #3's annual review of emergency contacts was last completed on 1/19/2023.
4. Child #6 enrolled early 2024 was missing proof of identity and results of the health examination.
