During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 04/07/2026 and completed on 04/07/2026. The on-site inspection began at 11:43am and ended at 12:54pm. The inspector reviewed compliance in the areas listed above. There were three children present and one caregiver; the point total was eight. The inspector reviewed five children?s records and two caregiver records on-site. This inspection included document review, tour of the facility, interviews.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 04/16/2026. A POC submitted after this date will not appear on the public website.

Repeat Violation:

The program is required to submit to VDH and implement a plan to test potable water for lead.

The program did not have evidence of a testing plan submitted to VDH.

Caregivers who work directly with children shall, in addition to preservice and orientation training required in subsections A through D of this section, annually attend at least 16 hours of training, to include the department's health and safety update course.

The health and safety update course for caregiver #1 and caregiver #2 was overdue by 23 days.

No soft bedding of any kind shall be used under or around infants including, but not limited to, pillows, quilts, comforters, sheepskins, or stuffed toys.

A stuffed toy and blanket was observed in the infant crib for child #1, age 9 months. Caregiver confirmed the use of the soft materials during nap time.

A child?s record shall contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.
