Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.
A monitoring inspection was initiated on 9/07/2021 and concluded on 9/09/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 60 children present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three children's records and three staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
2Evidence:
1. The record for child #1 listed the biological father listed as not authorized to pick up the child. There was no legal paperwork, as required, to support this.
Evidence:
1. Staff #2 had indicated on a Sworn Statement completed 8/1/2021 that they had lived in another state (AZ) in the past five years. There was no criminal record check or sex offender check conducted by the center for this staff person for that state of residence. The date of hire for staff #2 was 8/16/2021.
2. Staff #3 had indicated on a Sworn Statement completed 8/27/2020 that they had lived in another state (UT) in the past five years. There was no criminal record check or sex offender check conducted by the center for this staff person for that state of residence. The date of hire for staff #3 was 9/07/2020.
