Inspector Notes

An unannounced on-site renewal inspection was conducted on 12/15/2023. There were 71 children, ages 8 months-5 years, and 18 staff members present during the inspection. The inspector observed the following: free choice activities, and large movement outside. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: using the new orientation form found on the DOE website. The inspector arrived at 9:20 AM and departed at 11:30 AM.

Based on record review of five staff persons, the center failed to ensure that required background checks are repeated every 5 years per the Code of Virginia.

Evidence:

1.Staff # 2 had a fingerprint eligibility letter dated 10/30/2018. An updated fingerprint eligibility letter was not provided.
Plan of Correction: Licensee will schedule the staff person to have the updated finger prints completed ASAP.

Based on observation, the center failed to ensure that all toileting requirements were met.

Evidence:

Observed urine in a toilet chair in the toddler bathroom. All toilet chairs shall be emptied promptly and cleaned and sanitized after each use
Plan of Correction: The potty chair was emptied, cleaned and sanitized during the inspection.
