Inspector Notes
An unannounced virtual self-report inspection was conducted on 01/31/2024. Interviews were conducted with staff and staff record was reviewed. The inspector discussed the following with staff: a plan of correction with staff and director. The inspector arrived at 4 PM and departed at 4:30 PM

Based on interviews with staff and video recording, the center failed to ensure that physical punishment, striking a child, roughly handling or shaking a child, restricting movement through binding or tying, forcing a child to assume an uncomfortable position, or exercise as punishment.
Evidence:
The LI reviewed the video recording of an incident that occurred on 01/24/2024. Staff # 2 was observed sitting in a chair next to 5 two-year-olds playing. Child # 2 was playing with another child?s hair and staff # 2 moved from the chair to the floor and yanked child # 2 by her arm and into the staff?s lap. Staff # 2 confirmed during the interview that she did yank the child by her arm.

Plan of Correction: Per the Director:
1. She has been written up and will have a correction plan that will be in place once she writes it up and we agree upon it before her return.
2. She has been given a two day leave un-paid to come up with this plan.
3. She is to meet with the parents and talk about the incident.
4. She understands that she will be terminated immediately if such an occurrence happens again.
