Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 09/11/2025 and completed on 09/22/2025. The on-site inspection began at 10:00 AM and ended at 12:00 PM. The inspectors reviewed compliance in the areas listed above. There were 76 children present and 18 staff. The inspector reviewed 7 children?s records, 7 staff on-site. This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine.
Staff # 5 changed a child?s diaper in the infant room and did not wash the child's hands with soap and water.
Plan of Correction: In the future all infants hands will be washed with soap and water.
