Based on observation and interviews, it was determined that the facility did not ensure that the center notified the parent by the end of the day of any known minor injuries.
Evidence:
1.On 12/1/2023 at approximately 3:15PM child #1 was involved in an altercation with child #2
a.Child #1 hit his head on a door frame when he was pushed by child #2.
b. Following the incident child #1 was picked up by someone other than the parent.
2.The center did not call the parent to provide notification of the incident and injury.
3.The parent notified the center the following business day when child #1 complained that his head hurt. The child informed the parent of the incident.
4. The Director confirmed the center did not notify the parent.

Plan of Correction: Grandfather was notified upon pickup but moving forward a parent or legal guardian will be given a phone call or notified before the end of the day, not just a responsible adult at pickup to meet the standards.
