Inspector Notes

An unannounced inspection was initiated on 8/7/25 and completed on 10/10/25 in response to a complaint received by the licensing office on 7/30/25 relating to allegations of supervision and behavior guidance. The inspector reviewed 5 children's records and 5 staff records. This inspection included document review, interviews and observations. The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to records and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/22/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #1, who had been attending for seven months, did not have documentation of an immunization record on file.

Standard 8VAC20-780-160-A
Documentation of negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #1 had no documentation of a TB screening and had been employed for two months.

Staff who drive vehicles transporting children shall disclose any moving traffic violation that occurred five years prior to or during employment or assignment as a driver. Staff #3 and Staff #4, drivers for the facility, did not have documentation that any moving traffic violations were disclosed.

Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #3, enrolled for six months, was missing the house number and street of both emergency contacts. The record for Child #5 did not include the first day of attendance.
