Inspector Notes

An unannounced inspection was initiated on 10/7/25 and completed on 12/5/25 in response to a complaint received by the licensing office on 9/23/25 relating to allegations of supervision. The inspector reviewed two staff records on-site. This inspection included document review, tour of the facility, interviews and observations.The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Repeat Violation
The center must obtain a copy of the results of a central registry check. Staff #2, who had
been employed for five months, did not have central registry results in the record.
Plan of Correction: The central registry check has been completed.
Standard 8VAC20-780-350-B-1
The required ratio for children ages birth up to 16 months is one staff per four children. On 10/7/25 Staff #1 was the only staff member supervising up to 22 children. Child #1 (age: 5 months) and Child #2 (age:8 months) were present in the classroom
a. At 6:45am Staff #1 was alone, supervising 10 children.
b. At 7:18am Staff #1 was alone, supervising 17 children.
c. At 7:32am Staff #1 was observed alone, supervising 22 children.
Plan of Correction: The staff schedule had been re-evaluated and additional morning staff have been hired. The Director will continue to monitor to ensure that correct ratio's are maintained.
Standard 8VAC20-780-350-B-3
The required ratio for children who are two years old is one staff per eight children. On 10/7/25 at 7:44am Staff #2 was observed alone in a two-year-old classroom supervising 16 children (youngest child 2 years old).
Plan of Correction: The facility responded: The staff schedule had been re-evaluated and additional morning staff have been hired. The Director will continue to monitor to ensure that correct ratio's are maintained.
