Inspector Notes
An unannounced monitoring inspection was conducted on 4/30/2024 from 9:50 am-11:00 am. There were 12 children supervised by 4 direct caregivers. The home, 4 caregivers records, 2 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed outside engaging freely and participating in table top activities. Areas of non-compliance are identified in the Violation Notice. Violations
3Evidence:
1. There was no documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance for Child #1 and Child #2.
2. In the children's records, Child #1's start date is listed as 1/4/2024 and Child #2's start date is listed as 12/28/2023.
Evidence: A toaster oven was observed plugged in when not in use.
Evidence: A pressure-mounted gate was observed at the bottom of the stairway opening leading to the second floor.
