During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 10/23/2025. The on-site inspection began at 9:50AM and ended at 11:15AM. The inspector reviewed compliance in the areas listed above. There were 9 children present and 3 caregivers; the point total was 22. The inspector reviewed 5 children?s records, 3 caregiver and 1 household member records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
The provider shall obtain for each adult household member a current Report of Tuberculosis Screening every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

An updated TB screening for Household Member #1 was not completed and was overdue by seven months.
Plan of Correction: TB test completed on 10/23/2025.
Standard 8VAC20-800-280
Potentially poisonous substances, materials and supplies such as, but not limited to, disinfectants shall be stored away and inaccessible to children.

A container of Lysol disinfectant spray with the warning label, ?Keep out of reach of children,? was on the bench in the hallway, within reach of children.
Plan of Correction: Lysol spray was removed and placed in a locked cabinet during the inspection.
