Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

A monitoring inspection was initiated on August 3, 2021 and concluded on August 4, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were four children present for a total of eight points, ranging in ages from 15 months to 7 years of age, with two caregivers. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and three caregivers, and one household member's records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-111-210-A
Based on a review of staff records and training documentation, the provider failed to ensure that the 16 hours of training was obtained annually.

Evidence:
1. Caregiver #1's record only contained three hours of documented training for July 2020-June 2021.
2. The provider verified that Caregiver #1 had only three hours of documented training between July 2020-June 2021.
Plan of Correction: The provider indicated due to COVID they were having trouble locating trainings. The provider will research on-line trainings. Moving forward the provider will ensure that all caregivers have 16 hours of training annually.

Based on review of five children's records, the provider failed to obtain a parent's written acknowledgement of review of emergency information annually.

Evidence:
1. Child #1's last documentation that the emergency contact information had been reviewed was 07/16/2020.
2. Child #2's last documentation that the emergency contact information had been reviewed was 04/03/2020.
3. Child #3's last documentation that the emergency contact information had been reviewed was 07/16/202
Plan of Correction: The provider will contact the parents at pick-up and request them to review emergency contact information and obtain required signature.
