Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on January 19, 2022. There were three children present with three caregivers supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-665-230-E
Based on a review of caregiver's records, the provider failed to ensure that all caregivers had a current certification in CPR and First Aid.

Evidence:
1. Caregiver #2's CPR/FA certification expired on 11/3/2020.
2. The provider verified that Caregiver #2 did not have a current CPR/FA certification.
Plan of Correction: Caregiver #2 will find and obtain a CPR/FA certification as soon as possible. All caregivers in the future will keep CPR/FA certifications current and plan accordingly so that everyone is always current.
Standard 22VAC40-665-230-H
Based on review of caregiver records, the provider failed to complete the required annual health and safety update course.

Evidence:
1. Caregiver's #1, #2, and #3 did not complete the 3 hour annual health and safety training update for 2021.
2. The provider verified that the 3 hour training had not been completed for 2021.
Plan of Correction: All caregiver's will complete the Health and Safety training by 02/02/2022.
