A self-reported monitoring inspection was initiated on 3/12/24 and concluded on 3/12/24. The inspector reviewed compliance in the area of programming. The inspection started at 9:00am and concluded at 9:45am.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a self report by the center administration the center failed to ensure that rough handling of the children by staff does not occur.
Evidence:
Interviews and documentation supported that staff #1 grabbed child #1 on 3/7/24 by the upper arm, during nap time, leaving a scratch and what appear to be 4 nail marks. The marks and scratch are visible on the inner area of the upper arm.

Plan of Correction: The staff member was immediately placed on leave pending the outcome of the investigation. Administration promptly self reported to the appropriate agencies.
