Inspector Notes

An unannounced monitoring inspection was conducted from 2:00 - 3:00 pm with the provider. There were nine children, ranging from 12 months to four years of age, present with the provider and an assistant supervising. Children were observed napping during the inspection. One child was awake and playing with a variety of toys. Two children's records were reviewed and in compliance. Five caregiver/household member records were reviewed. The written attendance record, carbon monoxide detector and first aid kit were reviewed and in compliance. The provider is not Medication Administration Training (MAT) certified and cannot administer medication to children in care. Standard 22.1-289.036-A
Based on review of three household member records, it was determined that not all household members had a repeat central registry report every five years as required. Evidence: The record for household member #3 had a central registry search report dated: 12/28/2016.
Plan of Correction: Provider stated she will submit a request for an updated Central Registry search immediately.
Standard 8VAC20-770-60-C-2
Based on review of two caregiver records, and an interview with the provider, it was determined that not all caregivers had Central Registry Search results within thirty days of employment. Evidence: the record for staff #2 (Date of hire: 4/4/2022, did not have documentation of a Central Registry search report on file.
Plan of Correction: Provider stated that the wrong form was submitted and received. The corrected form will be used to obtain the missing search report.
Standard 8VAC20-800-900-B
Based on observation, and an interview with the provider, it was determined that children were allowed to drink while lying down. Evidence: two bottles were observed in cribs with children who were sleeping. The provider stated that there was milk in the bottles.
Plan of Correction: Bottles were removed during the inspection. The provider stated that bottles will not be allowed in the cribs.
