Inspector Notes
An unannounced renewal inspection was conducted on 11/9/2023. There were 6 children (12 points) directly supervised by 2 caregiver(s) The home, 2 caregiver records, 6 child's records, 4 household member's records, evacuation drills, emergency supplies, and policies were inspected. There were household medications on site today. Children were observed waking up from nap, preparing for and eating snack, playing freely outside and inside the play room. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Violations
4Evidence: reviewed 6 children's records and found that Child 2 did not have documentation of a signed physical exam and immunization record, and Child 4 did not have documentation of a signed immunization record.
Evidence: reviewed Household Member 3's record and found that the TB screening was a screenshot on a cell phone with no signature.
Evidence: observed that there were medications (Gabapentin and Aspirin) for household members in an unlocked bathroom cabinet, and a number of prescription medication bottles and over the counter medications on a top shelf of a dining cabinet, near the children's eating area and kitchen, that was not locked, and accessible to children.
Evidence: reviewed 6 children's records and found that Child 5 did not have documentation of reviewing or obtaining proof of identity.
