Inspector Notes
A monitoring inspection was conducted on 06/26/2024 from approximately 9:45 a.m. to 3:10 p.m. There were 22 children present ranging from age 3 to 5 years old and 4 staff supervising. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Five (5) records and five (5) staff records were reviewed. The information gathered during the inspection determined violations with applicable standards or law.
Please sign the violation notice and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

According to center documentation, child, C6, age 4, ?On Wednesday, April 17, 2024 a student, was found outside the classroom door unattended at 12:30. She was left unattended for approximately 10-15 minutes?. The center acknowledged the incident and completed at the time a review of the incident and supervision policy.

Evidence:
The written records for serious or minor injuries for C8 (06/20/2024), C9 (06/06/2024), C10
(06/06/2024) and C11 (06/06/2024) did not document the time when parents were notified
of the injuries and how the parents were notified of the injury. The elements on the written record were not completed.
Evidence:
Injury report for C7 dated 04/30/2024 documented the child requiring outside medical treatment. The center failed to inform the department of the injury.
