Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff #1 has not completed the pre-service training and has been working with children for more than 5 months. Staff #2 has not completed the pre-service training and has been working with children for 90 days.
Plan of Correction: Staff 1-Completed the 10 hour pre-service training on December 5, 2025. Certificate of completion attached. Staff 2-Erroneously completed the 3-hour renewal within 90 days of hire. The correct 10-hour pre-service course was completed on December 9, 2025. Certificate of completion attached.

Documentation of orientation training shall be kept by the center in a manner that allows for identification by individual staff member and is considered part of the staff member's record. In the record, there was no documentation that Staff #5 who has been working for more than 90 days, completed orientation training prior to working alone with children and no later than seven days of the date of assuming job responsibilities.
Plan of Correction: Employee and Head of School both recall the orientation being completed and appropriate documentation was signed: however the form could not be found. The orientation form will be recreated, signed and placed in the employee's file.
