Inspector Notes

An unannounced renewal inspection was conducted on 11/29/2022 between the hours of 11:25am-112:30pm. There were 4 children directly supervised by the provider and 1 assistant. The home, 2 staff records, 2 children?s records, 2 household member?s records, evacuation drills, and emergency supplies were inspected. There were no medications on site today. Children were observed playing freely working with caregivers to identify colors and count using flashcards. Diapering procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the provider did not obtain the results of a Central Registry check every 5 years for 1 caregiver.

Evidence:
1. The most recent Central Registry Check results in the record for Caregiver #1 were dated 11/20/2016.
2. Documentation in the record indicated a new request was sent on 8/31/2022 and was returned on 9/21/2022 due to incorrect information. No other results were in the record.
Plan of Correction: The document has been resubmitted and provider is awaiting results.

Based on record review, the provider did not obtain a current tuberculosis screening for one household member every two years.

Evidence:
1. The most recent tuberculosis screening results in the record for Household Member #1 were dated 11/11/2019.
Plan of Correction: I will have it completed this Saturday.

Based on record review, 1 of 2 childrn?s records reviewed did not contain documentation that a child has been adequately immunized before the child began attending the family day home.
