Inspector Notes

An unannounced monitoring inspection was conducted today from 9:40am-12:00pm. There were 8 children directly supervised by the provider and 2 assistant caregivers. The home, 3 caregiver records, 3 children?s records, 2 household member?s records, evacuation drills, and emergency supplies were inspected. Children were observed playing freely and having lunch. Areas of non-compliance are identified in the Violation Notice.

Standard 22.1-289.035-B-2
Based on record review, the provider did not obtain the results of a fingerprint-based criminal history record check on or before the first day of employment for 1 of 3 caregivers reviewed.

Evidence:
1. The provider stated Caregiver #2's first date of employment was 07/03/2023. The fingerprint check results in the record for Caregiver #2 were dated 08/07/2023.
Plan of Correction: Provider will ensure required background checks are obtained within required timeframes.
Standard 22.1-289.036-A
Based on record review, the provider did not obtain repeat background checks every 5 years for one caregiver and one household member.

Evidence:
1. The most recent central registry check results in the record for Caregiver #1 was dated 11/20/2018.
2. The most recent central registry check results in the record for Household Member #1 was dated 11/20/2018, The most recent fingerprint check results in the record for Household Member #1 were dated 10/20/2018. The most recent sworn statement or affirmation in the record for Household Member #1 was dated 11/01/2018.
Plan of Correction: The provider will submit documentation to the Department that the expired background checks have been submitted and/or scheduled within 10 business days.

Based on record review, the provider did not obtain a current report of tuberculosis screening at the time of hire for 1 of 3 caregivers.

1. The provider stated Caregiver #2 began employment on 07/03/2023. The tuberculosis screening results in the record for Caregiver #2 were dated 07/16/2023.
