Inspector Notes

An unannounced monitoring inspection was conducted today from 9:00am-10:00am. The home, 3 caregiver records, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed during storytime and playing freely.
Areas of non-compliance are identified in the Violation Notice.

Based on record review, the provider did not obtain repeat background checks every 5 years for one household member.

Evidence:
1. The most recent central registry check results in the record for Household Member #1 was dated 11/20/2018, The most recent fingerprint check results in the record for Household Member #1 were dated 10/20/2018. The most recent sworn statement or affirmation in the record for Household Member #1 was dated 11/01/2018.
Plan of Correction: Provider will obtain updated background checks for the household member.

Based on record review, the provider did not review annually with the parent, the emergency contact information to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.
