During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/19/2025, as a part of the licensure period. The on-site inspection began at 8:40am and ended at 9:45am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 16. The inspector reviewed 5 children?s records and 4 caregiver/household members' records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment. The criminal background check for Caregiver #2 was obtained more than one month after the employee's first date of employment.
Plan of Correction: Backgrounds have been obtained and placed in the record. Provider will request backgrounds within required time frames.
Standard 22.1-289.035-B-3
The center must request a search of the central registry prior to the employee's first day of employment. The central registry search request for Caregiver #2 was requested more than three weeks after Caregiver #2's first day of employment.
Plan of Correction: Backgrounds have been obtained and placed in the record. Provider will request backgrounds within required time frames.
Standard 8VAC20-800-170-B
Providers must obtain a current tuberculosis screening from a caregiver at the time of hire and prior to coming into contact with children. The form shall have been completed within the last 30 days prior to hire. The tuberculosis screening for Caregiver #2 was completed three months prior to date of hire. Caregiver #2 was present and providing care to children.
Plan of Correction: The provider will request a more recent screening.
Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for Household Member #2 was not completed and was overdue by four months.
Plan of Correction: The provider will request an updated screening.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review. The record for child #4 did not have a parent?s signature for an annual review of the emergency contact information. Child #4 had been in care for over a year.
Plan of Correction: The provider will request the parent review the information at pick up.
