Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol.
A renewal inspection was initiated on November 4, 2021 and concluded on November 16, 2021. The director was contacted via email and a remote inspection was conducted. There were seventy-one children present, with fifteen staff supervising and two administrators available for coverage. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records, five staff records, and three officer records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

1. The date of employment for Staff # 2 is 5/24/21. The results of a tuberculosis screening were dated 9/10/21.
2. The date of employment for Staff # 3 is 6/28/21. The results of a tuberculosis screening were dated 7/26/21.
3. Staff employed prior to 10/13/21 were required to obtain TB results within 21 days of beginning employment and such documentation was to be retained in the staff record.
